Please take a few minutes of your time to complete this survey. The information is used by the Family Resource Center to determine how we can best serve our students and families. The Family Resource Center offers programs and services for all students and families. This is possible thanks to the financing of the Cabinet of Health and Family Services. To obtain this funding, the center is required to conduct a Needs Assessment Survey to determine the programs and how the funds will be spent. This is an anonymous survey. All responses are confidential and the questions do not ask you to identify yourself or your student.
